Steps To Enrollment
This is what to expect before we can start services. Don’t worry if it seems complicated or overwhelming. We’ll guide you through each step. Please contact us with any questions.
1 - Intake Packet - Our office will send you an intake packet to complete. You must complete the health insurance form in order for us to contact your health insurance carrier on your behalf to request authorization for ABA assessment.
2 - Enrollment packet and the following items must be returned to our office:
Prescription/recommendation for ABA from your Primary Care Physician
Copy of diagnosis report
Copy of insurance card (front and back)
Copy of current IEP
Copy of previous assessments (psychological, behavioral, occupational therapy, speech, etc.)
3 - Assessment Process - A Loving Hands BCBA will complete a thorough assessment on your child’s current level of performance. The assessment process includes: parent/caregiver interviews, observations and direct assessments. The Assessment report will then be submitted to the funding source for authorization of ABA services.
4. Starting Services - During the assessment process, we will be working on building a treatment team for your child. This will consist of a Supervisor that will oversee your progress and one or more Behavioral Technicians that will perform the 1-on-1 treatment.
*All of our Supervisors are Board Certified Behavior Analysts with thousands of hours of fieldwork experience and Master’s degrees or higher in Education or Psychology.
